Before September 6th, 1986[]
As Gary Miller operates a local clinic, the Department of Health suddenly appears to perform a surprise inspection of the place. Gary and his cult apparently did a good enough job at hiding their true intentions to avoid having the clinic shut down.
In a later fax to his secretary, Gary informs her not to return any calls from the Department of Health before notifying him or leaving a message with Tiffany first, fearing that another surprise inspection would jeopardize their operation.

- It is reasonable to assume that the Department of Health's surprise inspection of the clinic took place before September 6th, 1986, as Gary's same fax instructs Rhonda to let him know if she hears back from Amy Martin. This likely means that Amy had not yet been possessed by the cult, which would begin her journey into becoming a vessel for the Second Death.
