The ritual mask is an object that appears in FAITH: Chapter III.
It is to be placed on the victim's face during the Second Death ritual, which is then carved out by the sacrificial dagger.

Presumably, the mask is used by the Eternal Order of the Second Death while administering the Second Death ritual onto many vessels.
October 29th, 1987[]
John Ward is tasked with heading to the New Haven Apartment Complex to help his childhood friend, Lisa Pearson. Upon arriving, the door to her room is blocked by the Seal of Alu, which John must exorcise the mask and dagger to remove.
After finding the ritual mask in the basement beside the site of a ritual, John exorcises it to remove half of the seal on Lisa's door. Exorcising the dagger will remove the second half.
If John forgets to exorcise the seal itself first, neither the mask nor the dagger can be exorcised.
October 31st, 1987[]
The mask and dagger appear mounted on the wall of Garyland during the showdown with Gary, who plans to use them on John.
