Survival Mode is an unlockable game mode found in the "Extras" menu.

In this game mode, John Ward has to navigate through the Yale Psychiatric Institute, reliving every demon fight from the main game one after the other, in order of appearance.

The mode is unlocked after getting the good endings in all three chapters of the main game, and completing the Chapter II Prologue.

Enemies[]

Every fightable enemy that appears in the three chapters and Chapter II Prologue appear in this mode. Quite a few changes have been made to the enemies in Survival mode in comparison to the regular game mode. Some changes are significant stats and mechanics changes, while others are small like the sizes and shapes of the rooms you're fighting in.

Michael[]

Unlike in the main game, Michael quickly spawns again after scaring him away, without needing to move to a different area. After scaring him away he will spawn again in a random place after about 1 to 5 seconds. Upon scaring Michael away 10 times, the next area to the north becomes accessible, and the demon kill counter is increased by +1.

Amy's Ghost[]

Amy's Ghost will appear at the bottom of the screen from past the entrance, slowly approaching John. She only needs to be defeated once to open the next door and count as +1 demon killed.

Amyghost survivalmode

Amy[]

Amy acts very similar to her main game version, however her stats in survival are as if you have collected all the notes. Her first phase has only 200 hp, and she spawns the first Gray Demon when at 199 hp, and the second Gray Demon when at 100 hp. However, it's easy to kill her fast enough that the second Gray Demon won't even be visible on screen. Her second phase has 200 hp as well and is identical to the one in the main game. Phase 3 has 200 hp too and also the same amount of speed (35), as if you collected all the notes. In total she has 600 hp. During this fight, cutscenes don't play and instead she transitions to next phase instantly. After completing Phase 3, killed demons counter is increased by +1.

Mirror Demon (Chapter I)[]

The first Mirror Demon fight is identical to one in Chapter I, except for the room you're fighting it in. Counts as +1 demon killed.

Mirrordemon survivalmode

Cornfield Maze Demon[]

The Cornfield Maze Demon fight is nearly identical to the one in the Chapter II Prologue, however, unlike in the main game you don't need to go to different rooms to spawn the demon again, and instead Demon will spawn three times in the same room until defeated. Counts as +1 demon killed.

Corndemon survivalmode

Thrall[]

A thrall hides behind a tree and runs towards John as he approaches the door. Warding off the thrall with the cross will count as +1 demon killed.

It is possible to reach the door without warding off the thrall, missing out on the kill. For the best performance, make sure to scare them away before proceeding.

Chalice Demon[]

The Chalice demon fight is identical to one in Chapter II Prologue, except for the room you fight in. Counts as +1 demon killed.

Basement Demon (Chapter II Prologue)[]

The first Basement Demon fight is identical to one in Chapter II Prologue, with the exception of the room. Counts as +1 demon killed.

Basementdemon1 survivalmode

Cemetery Trinity[]

John comes across a room with three open doors and a trapdoor in the center. The Mist Demon, Umbilical Cord Demon, and Graveyard Maze Demon can be fought in any order, and all must be killed for John to continue through the trapdoor.

The fights are identical to those in Chapter II, except for rooms you fight them in. Each demon counts as +1 demon killed, giving a total of +3.

Pentarox[]

The Pentarox fight is identical to the one in the Main Game. Counts as +1 demon killed.

Mirror Demon (Chapter II)[]

The second Mirror Demon fight is identical to the one in Chapter II. Counts as +1 demon killed.

Confessiondemon survivalmode

Spindly Lady[]

The Spindly Lady fight is identical to the one in the main game, apart from the room you're fighting in. Counts as +1 demon killed.

Spindlylady survivalmode

Peekaboo Demon (Chapter II)[]

The first Peekaboo Demon encounter is same as the one in Chapter II, except for the room. A cutscene will play when the demon gets close to John. He must not move until the demon has completely left the room. Counts as +1 demon killed, despite not actually having exorcised it.

Peak1 survivalmode

Basement Demon (Chapter II)[]

The second Basement Demon fight is identical to the one in Chapter II, with the exception of the room. Also, it appears that the Martin Twins do not appear as distractions in this fight, like they do in the Chapter II fight. Counts as +1 demon killed.

Basementdemon2 survivalmode

Blind Ones[]

The blind ones room contains 9 enemies that John must avoid. He only needs to continue walking north to get to the next fight. Just be careful that you know where all the demons are, as they can easily surround you from all sides. Unlike other encounters, the demons killed counter does not increase when completing this room.

Miriam Bell[]

The Miriam fight is slightly different from the one in Chapter II. Father Garcia is absent, so John will have to exorcise Miriam himself. Not only that, but just like if you're doing the bad ending, Miriam can Morph into a more stretched and grotesque version of her sprite. However, unlike in the main game, this morphing has a gameplay effect, making it so that the next phases have more hp than usual. Miriam will always have 800 hp in Phase 1. In Phase 2 she will have 60 hp if she not morphed, 120 if she has morphed once and 180 if she has morphed twice. In phase 3 she will normally have 100 hp if she hasn't morphed, 200 hp if morphed once and 300 if morphed twice. Her total hp is 960 if she morphed 0 times, 1120 if she morphed once and 1280 if she morphed twice. In every other way, the fight is exactly same, except for the lack of cutscenes. Counts as +1 demon killed upon finishing her third phase.

Miriam survivalmode

Jeffrey[]

The Jeffrey fight is quite different to the Chapter III fight, since you will not have the assistance of the Police Officer. Instead, a rifle is placed on the ground which John may use to shoot Jeffrey.

Jeffrey has four rooms in total, however you are only required to shoot him in the last room to progress; John is able to proceed to the last room without shooting him the first three times.

Shooting Jeffrey in each room counts as +1 demon killed. In order to get the maximum demons killed score, Jeffrey must be shot each of the four times, giving a total of +4 demons killed.

Mother and Child[]

The Mother and Child boss fight is nearly identical to one in Chapter III, with the exception of the room geometry. Counts as +1 demon killed.

The babies do not count towards the number of demons killed.

Mommy survivalmode

Elevator Friend[]

The Elevator Friend encounter is similar to one in Chapter III, however you only have to defeat him once. It's possible that the demon kill is not counted here if you exit the room before Elevator Friend despawns.

Peekaboo Demon (Chapter III)[]

The second Peekaboo Demon fight is identical to one in Chapter III, with the exception of where you and the demon start and the room you're fighting in. Counts as +1 demon killed.

Peak2 survivalmode

Alu[]

The Alu boss fight is very similar to one in Chapter 3, however stick figures are places differently and there is also a tank of liquid in the center of the room.

Exorcising Alu out of the room counts as +1 demon killed. If John lets Lisa die during this fight, the counter does not increase.

Tiffany[]

Tiffany's fight is identical to the one in Chapter III, except there are no cutscenes and you are not chased by her remains when you enter the next room. Counts as +1 demon killed.

Plagas Cultist[]

The plagas cultist encounter is a room with 3 Plagas' slowly moving towards John. The rifle makes a reappearance here, allowing John to get an easy kill on one of them. This is considered a very difficult room to complete due to the random nature of this enemy. Killing all three cultists counts as +1 demon killed.

Unholy Spirit[]

The Unholy Spirit fight is very similar to the one in Chapter III, except for the fact that leaving the Room to evade the demon leads back into the same room, which is easier than the base game fight because this room is a lot larger and easier to maneuver in.

Unlike in Chapter III, the Unholy Spirit will kill John instantly on contact without sending him to the mental asylum area (because he is already there).

Counts as +1 demon killed.

Demonhead survivalmode

Mirror Demon (Chapter III)[]

The final Mirror Demon fight is similar to one in Chapter III, however the mirrors' placement and room size is different. Counts as +1 demon killed.

When the Mirror Demon is defeated, the mirrors will shatter, and 4 Malphas Acolytes will appear behind it and start chasing John. Killing them counts a +1 demon killed, giving a total of +4 demons killed.

Mirrordemon2 survivalmode

Malphas Acolytes[]

In this room, a large horde of Malphas Acolytes appear around you and move towards John. There are 13 in total that must be defeated. One good strategy is to walk around and get them in 1 big group, and then exorcise them all at once. Counts as +1 demon killed.

Horror[]

The Horror fight is similar to the one in Chapter 3, with the difference being the room you fight in. Counts as +1 demon killed.

Gary Miller[]

The Gary fight isn't any different for the one in Chapter III, except for the room you fight in and lack of cutscenes. Counts as +1 demon killed.

There is a 1/8th chance that when Gary uses his "Rain of Spiders" attack, he will say "AUSTRALIA" instead.

Malphas and Astaroth[]

The Malphas and Astaroth fight is similar to the one in Chapter III, however, the room does not include Miriam's body or any torches, allowing for full mobility throughout the room, which makes avoiding seals slightly easier. Counts as +2 demons killed.

Malphas astaroth survivalmode

Super Miriam[]

The Super Miriam fight is nearly identical to the one in Chapter 3, however the fight starts when you get close to the red door and you also don't have to set Miriam on fire, as she dies automatically when her hp reaches 0. Counts as +1 demon killed.

Survival mode results

After completing Survival Mode, the player's statistics will be shown, displaying the time it took them to beat the game, how many demons they defeated, and how many times they died. They will also be given a rank based on the following conditions, from worst to best:

Altar Boy Take 90 minutes or more to beat the game.
Alma Perdida Beat the game in under 90 minutes.

Never appears due to a programming oversight.

Choir Boy Beat the game in 70-89 minutes.
